We offer several ways for you to review and request a copy of your medical records.

You may visit any Patient First center with your current identification to receive a paper copy of (i) your medical records for a single visit or limited number of related visits, and/or (ii) itemized invoices for individual dates of service. Copies of digital x-rays are also available on disc.

By Mail or FAX

To request a copy of your medical records, click here to download and complete the release of information form. Once completed, the form can either be (i) mailed to the address below, or (ii) faxed to the fax number below.
